To create an Guest Book, use this Guest Book template in Google Docs.

• Edit this template in Google Docs and add any additional questions you'd like.

• Once you're done, add the form to this page

  1. Click "Edit Page"

  2. Select "Insert" and the select "Spreadsheet form"

  3. Select the form you just created form the list.

  4. Click "Save."
